目的 观察生物陶瓷材料iRoot BP Plus用于成熟恒牙不可复性牙髓炎活髓切除术的疗效。方法 选择2022年3月至2024年3月西安交通大学口腔医院收治的成熟恒牙不可复性牙髓炎患者100例(100颗患牙)为研究对象,按照简单随机法分为对照组和试验组,每组50例(各50颗患牙)。对照组使用三氧化矿物凝聚体(MTA)盖髓剂行活髓切除术,试验组使用生物陶瓷材料iRoot BP Plus盖髓剂行活髓切除术。比较两组患者疼痛程度、临床疗效、影像学疗效及牙齿变色率。结果 术后第1天和第7天,两组患者视觉模拟评分法(VAS)评分均较术前降低(P<0.05),术后第7天较术后第1天下降(P<0.05),且术后第1天和第7天试验组VAS评分低于对照组同期(P<0.05)。术后3、6、12个月,试验组临床成功率分别为96.00%、94.00%、94.00%,对照组分别为94.00%、92.00%、90.00%,两组比较,差异均无统计学意义(P>0.05)。术后3、6、12个月,试验组影像学成功率分别为96.00%、92.00%、92.00%,对照组分别为92.00%、90.00%、90.00%,两组比较,差异均无统计学意义(P>0.05)。术后12个月,试验组牙齿变色率为2.00%(1/50),低于对照组的46.00%(23/50)(χ2=26.535,P<0.05)。结论 在成熟恒牙不可复性牙髓炎的活髓切除术中,使用生物陶瓷材料iRoot BP Plus的短期和长期疗效与MTA相当,但在减轻术后疼痛和避免牙齿变色方面更具优势。
Abstract:Objective To observe the efficacy of the bioceramic material iRoot BP Plus in vital pulpotomy for irreversible pulpitis of mature permanent teeth. Methods A total of 100 patients(100 affected teeth) with irreversible pulpitis of mature permanent teeth admitted to the School of Stomatology, Xi'an Jiaotong University from March 2022 to March 2024 were selected as the study subjects. They were divided into a control group and an experimental group by simple randomization, with 50 cases in each group(50 affected teeth each). The control group underwent pulpotomy using mineral trioxide aggregate(MTA) as the pulp capping agent, while the experimental group underwent pulpotomy using the bioceramic material iRoot BP Plus as the pulp capping agent. Pain level, clinical efficacy, radiographic efficacy, and tooth discoloration rate were compared between the two groups. Results On the first and seventh days after surgery, the visual analog scale(VAS) scores of both groups decreased compared with those before surgery(P<0.05), and the scores on day 7 were lower than those on day 1(P<0.05). The VAS scores of the experimental group were lower than those of the control group on both day 1 and day 7 after surgery(P<0.05). At 3, 6, and 12 months after surgery, the clinical success rates of the experimental group were 96.00%, 94.00%, and 94.00%, respectively, while those of the control group were 94.00%, 92.00%, and 90.00%, respectively, with no statistically significant differences between the two groups(P>0.05). At 3, 6, and 12 months after surgery, the radiographic success rates of the experimental group were 96.00%, 92.00%, and 92.00%, respectively, while those of the control group were 92.00%, 90.00%, and 90.00%, respectively, with no statistically significant differences between the two groups(P>0.05). At 12 months after surgery, the tooth discoloration rate of the experimental group was 2.00%(1/50), which was lower than that of the control group at 46.00%(23/50)(χ2=26.535, P<0.05). Conclusion In vital pulpotomy for irreversible pulpitis of mature permanent teeth, the short-term and long-term efficacy of iRoot BP Plus is comparable to that of MTA, but it shows advantages in reducing postoperative pain and preventing tooth discoloration.
